I need help making a babysitting flyer?

I'm making a babysitting flyer. Its done, but the only thing I want to do is make tabs at the bottom of the page with my name and number so they can tear them off if they plan to contact me later. I dont want to have to handwrite them I want to type them. I dont know how though. I have microsoft works word processor. Could someone explain to me how to do it?
